MEMORIAL PLAQUES
In order to mark the historical and strategic significance of the Samford Avenue of Honour, we are offering memorial plaques to honour deceased Service Personnel, to be installed in the pathway along the Avenue of Honour.
The intention is to honour members of the defence forces who have served in the defence of Australia (Free World) of any nationality in times of war and to honour members of the Australian Defence Forces in times of peace. The importance of Samford (and surrounding areas) was enormous and deserves to be fully and suitably recognised and respected.
The cost of these plaques is $300 per plaque and covers manufacture and installation of the plaque.
Anyone wishing to be involved in this historical project is invited to do so by completing the attached Application form and returning it with a Discharge Certificate or other proof of identification containing the person’s name, rank, service number and unit, squadron or ship etc together with payment (cheques or Money Orders are to be made payable to Samford Avenue of Honour) and all mailed to P O Box 175, Samford Qld 4520.
The Office of Australian War Graves wishes to advise that due to the increasing number of requests for Permission to Use the Service Emblem, applications are now required to include a copy of one of the following:
· Discharge Certificate
· Statement of Service
· WWI Nominal Roll sheet printout which can be accessed at www.awm.gov.au
· WWII Nominal Ross printout which can be accessed at www.ww2roll.gov.au
· Korean Nominal Roll printout which can be accessed at www.koreanroll.gov.au
· Vietnam Nominal Roll printout which can be accessed at www.vietnamroll.gov.au
· Any Official Document showing the service number or service details.
